Welcome to Yonge Urgent Care Clinic — we’re delighted to have you as our patient.

Please review the following guidelines before submitting your registration form or booking an appointment.

New Patient Registration

• Complete the online registration form carefully with accurate personal details.

• Upload or bring a copy of your health card (OHIP) or other valid ID.

• For uninsured patients, please mention your payment preference (cash, debit, or credit).

• You will receive a confirmation message once your registration is processed.

For OHIP-Covered Patients

• Ensure your OHIP card is valid and not expired at the time of your visit.

• Please present your card at each visit for verification.

• Some services (e.g., forms, cosmetic procedures, travel medicine) are not covered by OHIP and will be billed privately.

For Uninsured / Private Patients

• You are welcome to register and book appointments as a private patient.

• Payment is required at the time of service.

• A receipt will be issued for insurance or tax purposes.

Appointment Scheduling

• Once registered, you can book an appointment online, by phone, or walk in (subject to availability).

• Same-day or next-day appointments are usually available for urgent or walk-in care.

• Please arrive 10–15 minutes early for your first visit to complete any remaining paperwork.

Updates & Follow-Ups

• Inform the front desk if you change your address, phone number, or family doctor.

• For follow-up visits, bring any test results, referral letters, or medication lists.

• Please notify us at least 24 hours in advance if you need to cancel or reschedule your appointment.

Privacy & Confidentiality

• All patient information is kept strictly confidential in accordance with the Personal Health Information Protection Act (PHIPA).

• Your data is used only for medical and administrative purposes.

What to Bring on Your First Visit

✅ Valid health card or photo ID
✅ List of medications or allergies
✅ Any previous medical reports (if relevant)
✅ Insurance details (if applicable)
