Booking Options

You may book your appointment through any of the following:

🚶‍♂️ Walk-In: Accepted based on physician availability.

Please include your full name, date of birth, contact number, and reason for visit when booking.

Appointment Confirmation

After submitting your request:

• Our staff will review your details and contact you to confirm your date and time.

• Appointments booked after hours will be confirmed the next business day.

What to Bring

✅ Valid OHIP card or photo ID

✅ List of current medications/allergies

✅ Previous reports or referrals (if applicable)

✅ Payment method (for private/uninsured services)

Arrival & Check-In

• Arrive 10–15 minutes early for registration and verification.

• Late arrivals may need to be rescheduled to avoid delay for others.

Cancellations & Rescheduling

• Please notify us at least 24 hours in advance if you need to cancel or change your appointment.

• Missed appointments or same-day cancellations may result in a cancellation fee (for private services).

Follow-Up or Virtual Visits

• If your doctor advises a follow-up, please schedule it before leaving or via this form.

• Virtual or phone appointments are available for select consultations.
