Additional Services & Uninsured Fees
Some services are not covered by OHIP and may require direct payment. The sections below outline common administrative, procedural, and documentation-related fees for your reference.
Missed or Late Appointments
- Missed or Late Appointment Fee – $40 (Applies to missed appointments, cancellations with less than 24 hours’ notice, or arriving more than 10 minutes late. Fee must be paid before future bookings.)

Forms & Notes
