Booking Your Appointment
You may access care through the following options:
- Online Booking – Use our online form to schedule your visit.
- By Phone – Call our reception during business hours.
- Walk In – Accepted based on physician availability.
Required Information
When booking an appointment, please provide:
- Full Name
- Date of birth
- Contact number
- Reason for visit
Appointment Confirmation
- Our staff will contact you to confirm your appointment.
- Requests submitted after hours will be confirmed on the next business day.
Arrival Guidelines
- Please arrive 10–15 minutes early for registration and verification.
- Late arrivals may need to be rescheduled depending on clinic capacity.
Registration Requirements
Before your first visit:
- Bring a valid OHIP card or government-issued ID.
- Ensure your contact details are up to date.
For Uninsured Patients
- Indicate your preferred payment method (cash, debit, or credit).
- Fees will be discussed prior to consultation where applicable.
What to Bring
To ensure efficient care, please bring:

- List of medications and allergies
- Relevant prior reports or documents

Cancellations & Rescheduling
- Kindly provide at least 24 hours’ notice for cancellations or changes.
- This allows us to accommodate other patients in need.
Follow-Up & Virtual Visits
- Follow-up appointments can be arranged before leaving the clinic.
- Virtual or telephone consultations are available for appropriate cases.
Walk-In Clinic Details
No appointment? No problem. Our clinic welcomes walk-in patients during regular operating hours and provides compassionate care for everyone.
Here’s what you need to know for your walk-in visit:
- No appointment is needed for routine walk-in services.
- Our current walk-in hours are consistent with our general clinic hours.
- Walk-in patients are seen on a first-come, first-served basis, so we encourage you to arrive early if possible.
- Please bring your health card and a list of current medications.
- If you have any query, our staff will be happy to assist you upon arrival.
